How to Submit a New User Form

Home Screen

The Home screen has 2 buttons

• New Form – for submitting new user forms

• View My Forms – for viewing your previously submitted forms

• My Forms screen will show ‘getting your Data’ if you have not submitted a form before

Submit Form Screen

• Cancel – returns to Home screen

• The Submit Form button will become active once all required fields are filled in

• Reset Form – Clears form

My Forms screen

• Here you can view and edit forms previously submitted by yourself

• The Save button is only enabled for forms with a status of New, to allow any edits or additions prior to account creation.

• If you need to update a record, make the changes, and hit the save button.

• Search your Forms using First or last name.
